What to include with the order
- Patient information: full name, date of birth, phone number, and home address
- Due date: estimated due date, or delivery date if she's already postpartum
- Diagnosis: ICD-10 diagnosis code(s) supporting the item ordered
- Insurance card: copy of the front and back
- Prescriber signature: signed and dated, with printed name and NPI
